Each line is a provider whose answer on an industry changed in our base, with the day we recorded it. A change can be a new rule on the provider’s page or one we had missed until we read it again; the corrections page says which.

Based on providers’ published policies. No provider pays for placement. Facts carry the date they were last verified. Policies change; re-check before applying if a date is older than 90 days
